This page provides a list of the FHIR artifacts defined as part of this implementation guide.
Decedent Demographics is a profile group comprised of profiles containing decedent demographic data. Demographic data includes information about related persons (Mother, Father, and Spouse) and social determinants such as education level and usual occupation. The information in this profile group are provided by a decedent informant.
VRDR Decedent |
The Decedent’s Education Level resource represents the highest level of education achieved by the decedent. The VRDR Decedent’s Education Level profile builds upon the FHIR Observation resource. |
VRDR Decedent Age |
The Decedent Age resource represents the decedent’s chronological age at the time of death. Age is derived as the difference in time between the decedent’s death date and birth date. The VRDR Decedent Age profile builds upon the FHIR R4 Observation Resource. |
Decedent's Mother |
The Decedent’s Mother resource represents the maternal parent of the decedent. The VRDR Decedent Mother profile builds upon the FHIR R4 Related Person resource. |
Decedent Spouse |
The Decedent Spouse resource represents the surviving spouse of the decedent. The VRDR Decedent Spouse profile builds upon the FHIR R4 Related Person resource. |
Birth Record Identifier |
The Birth Record Identifier resource is a reference to a birth record used as a linkage. The VRDR Birth Record Identifier profile builds upon the FHIR R4 Observation resource. |
Decedent's Usual Work |
The Decedent Usual Work resource is used to convey the usual work industry and occupation of the decedent. The base profile is lifted directly out of the Occupational Data for Health (ODH), Release 1.0 FHIR Implementation Guide (http://hl7.org/fhir/us/odh/). |
Decedent Military Service |
The Decedent Military Service resource is used to convey whether the decedent was ever in the US armed forces. The VRDR Decedent Military Service profile builds upon the FHIR R4 Observation resource. |
Decedent Father |
The Decedent’s Father resource represents the paternal parent of the decedent. The VRDR Decedent Father profile builds upon the FHIR R4 Related Person resource. |
Decedent |
The decedent resource represents the deceased person that is the subject of the vital record death report. The VRDR Decedent profile builds upon the US-Core Patient profile. |
Death Investigation is a profile group comprised of profiles containing data obtained during the course of investigating a death. Many of these items are conditionally present in a death certification transaction depending on whether or not a autopsy was performed, an injury incident occurred, or a transportation event was involved.
VRDR Examiner Contacted |
The Examiner Contacted resource represents an indication as to whether a medical examiner (or coroner) was contacted to conduct an investigation and certify the causes of death. The VRDR Examiner Contacted profile builds upon the FHIR R4 Observation resource. |
VRDR Decedent Pregnancy |
The Decedent Pregnancy resource provides an indication of the pregnancy status of the decedent. The VRDR Decedent Pregnancy profile builds upon the FHIR R4 Observation resource. |
VRDR Decedent Transportation Role |
The Decedent Transportation Role resource is used to specify the role of the decedent (e.g. driver, passenger, pedestrian, other) in a transportation-related injury. The VRDR Decedent Transportation Role profile builds upon the FHIR R4 Observation resource. |
Tobacco Use Contributed To Death |
The Tobacco Use Contributed To Death resource represents the conclusion arrived at during death investigation regarding the extent to which tobacco use contributed to the death of the decedent. The VRDR Tobacco Use Contributed To Death profile builds upon the FHIR R4 Observation resource. |
Injury Location |
The Injury Location resource represents the general place (such as restaurant, vacant lot, or home) where a decedent’s death-related injury occurred. The VRDR Injury Location profile builds upon the FHIR R4 Location resource. |
VRDR Autopsy Performed Indicator |
The Autopsy Performed Indicator profile provides an indication as to whether an autopsy was performed and if so, indicates whether the results of the autopsy are available. The Autopsy Performed Indicator profile builds upon the FHIR Observation resource. |
VRDR Death Location |
The Death Location resource represents the decedent’s place of death. The VRDR Death Location profile builds upon the FHIR R4 Location resource. |
Death Pronouncement Performer |
The Death Pronouncement Performer resource represents the health practitioner that declared the decedent dead. The VRDR Death Pronouncement Performer profile builds upon the FHIR R4 Practitioner resouce. |
VRDR Injury Incident |
The Injury Incident resource represents observations (time, location, and work or transportation-related), discovered regarding an injury event during a death investigation. The VRDR Injury Incident profile builds upon the FHIR R4 Observation resource. |
Death Date |
The Death Date resource is used to convey the actual or presumed date of death for the decedent. If the date of death unable to be determined then the date the decedent’s remains were discovered serves as a surrogate for the death date. The VRDR Death Date resource profile builds upon the FHIR R4 Observation resource. The date the decedent was pronounced dead is included as a component of the death date observation. |
Death Certification is a profile group comprised of profiles containing data related to the primary concern of the VRDR IG, Death Certification. In this group the causes of death and the causal pathway are documented and attested to by a death certifier (coroner or medical examiner).
Interested Party |
The Interested Party resource represents a business entity that assigns one or more identifiers to a death certificate document. Assigning authorities for death certificate identifiers include coroners and medical examiners, local and state public health agencies, and the national center for health statistics. The VRDR Interested Party profile builds upon the FHIR R4 Organization resource. |
VRDR Manner of Death |
The Manner of Death resource represents the conclusion arrived at upon completion of death investigation regarding the manner of death (eg., natural, homicide, suicide, accident, …). The VRDR Manner of Death profile builds upon the FHIR R4 Observation resource. |
Cause Of Death Condition |
The Cause Of Death Condition resource represents a clinically relevant condition –disease, injury, or complication– in the causal pathway of events that directly caused the death. The VRDR Cause of Death Condition profile builds upon the FHIR R4 Condition resource. |
Certifier |
The Certifier resource represents the practitioner (coroner, medical examiner, or physician) who certifies death, the manner of death, and the causes of death. The VRDR Cerfifier profile builds upon the FHIR R4 Practitioner resource. |
VRDR Condition Contributing To Death |
The Condition Contributing To Death resource represents a significant condition contributing to death but not resulting in the underlying cause. The VRDR Condition Contributing To Death profile builds upon the FHIR R4 Condition resource. |
VRDR Death Certificate |
The Death Certificate resource represents the body of the death certificate document that lays in order to the bundle of resources comprising the death certificate document. The VRDR Death Certificate profile builds upon the FHIR R4 Composition resource. |

The Death Certificate Reference resource enables interested parties to assign local identifiers to the death certificate document. The Death Certificate Reference profile builds upon the FHIR Document Reference resource. |
Decedent Disposition is a profile group comprised of profiles containing data related to the disposition of the decedents remains. The data in this profile group is typically provided by the funeral home performing the disposition services. The content of this profile group is authenticated by the funeral home director.
Disposition Location |
The Disposition Location resource represents the place/location where the decedent’s remains were disposed of. The VRDR Disposition Location profile builds upon the FHIR R4 Location resource. |
Funeral Home |
The Funeral Home resource represents the business entity conducting ceremony connected with the final disposition of the decedent’s remains, such as a burial or cremation, with the attendant observances. The VRDR Funeral Home profile builds upon the FHIR R4 Organization resource. |
Mortician |
The Mortician resource represents the professional practitioner preceding over the funeral services involved in the disposition of the decedent’s remains. The VRDR Mortician profile builds upon the FHIR R4 Practitioner resource. |
Decedent Disposition Method |
The Decedent Disposition Method resource specifies the means by which the decedent’s remains were disposed of. The VRDR Decedent Disposition Method profile builds upon the FHIR R4 Observation resource. |
Funeral Service Licensee |
The Funeral Service Licensee resource represents the professional practitioner affiliated with the funeral home that has met the requirements to be a funeral director and/or an embalmer. The VRDR Funeral Service Licensee profile builds upon the FHIR R4 PractitionerRole resource. |

VRDR Within City Limits Indicator |
The Within City Limits Indicator resource is an extension to the FHIR Address Datatype. It is used to indicate if the decedent’s residential address is a location within the limits of the address city. |
Observation Location |
Observation Location is an extension of the FHIR Observation resource. It is the location of the observed phenomenon or entity that is the subject of the observation. Use to enable the specification of locations such as the location of injury, the location of death, and the location of disposition. |
Location Jurisdiction Id |
An extension intended for use in an FHIR Location resource to identify the national case reporting jurisdiction under which the location falls. The jurisdiction identifier can be express as a string or as a code value from the PHVS_USVitalRecordsJurisdictions_NCHS value set. |
Extension - Partial Date Part Absent Reason |
This extension enables an incomplete date to be expressed with reasons provided for missing date components. |
Bypass Edit Flag |
An extension used to indicate the rationale behind the inclusion of data values that bypass edit criteria for the resource element. |
Address City Code |
The address city code extension is used to enable the city portion of address to be expressed as a numeric value in accordance with the NCHS Instruction Manual Part 8, Vital Records Geographic Classification, 2014 (https://www.cdc.gov/nchs/data/dvs/IMP8_2014.pdf). |
NVSS Ethnicity |
An extension used to indicate the ethnicity of a decedent. |
